Klein ISD Medication Policy and Form

***Form for all prescription medications MUST be signed by your doctor and a parent/guardian!***

Over the Counter Medications:

***The clinic does not carry stock medication. Medication must be supplied by parent/guardian.***

  • Non-prescription/over the counter medications such as Tylenol, Advil, etc. can be kept and administered in the clinic for a two week period without a doctor's note. All medication must be age appropriate, with expiration date within limit and the parent must provide a dated, signed note with specific instructions. Over the counter medications will be kept in the clinic for two week only, then sent home with the student.
  • Over the counter medications needing to be kept and administered in the clinic longer than a two week period need to be approved by a doctor. Please use the Medication Authorization Form available on Klein's website. Both parent and physician signatures are required.
  • Students MAY CARRY non-prescription/over the counter medications such as Tylenol, Advil, etc. with a dated, signed parent note that is kept with the medication stating permission to self-administer. All medication must be in a small original bottle, age appropriate, with expiration date within limit. Students are strongly reminded NOT TO share medication and must take the medication in the clinic. The consumption of pills in any form on campus may result in being mistakenly charged with a violation of the district's policy on drug abuse.
